Scheduling Appointments

Use this link to book a visit with any member of your MEDgevity care team — including your physician, registered dietitian, or exercise physiologist.

Once scheduled, you’ll receive a confirmation email with your virtual visit link and the option to add the appointment to your calendar (which we highly recommend for convenience!).

To help your MEDgevity dietitian better understand your eating habits, preferences, and overall nutrition patterns, they may ask you to log your meals for a few days. You can do this using the form below:

Frequently Asked Questions

  • Your virtual visit link will be included in your confirmation email and in the reminder emails sent 24 hours and 1 hour before your appointment.

  • Text or call us at 954-420-1145 at any time, and we’ll be happy to help you reschedule or cancel your visit.

  • You can securely upload documents here, or simply text a photo or PDF to us at 954-420-1145. We’ll make sure your care team receives and reviews the information as part of your medical record.

  • Yes! Just fill out our medical records release form here, and we’ll be happy to share your records with your other provider as requested.

  • No problem! You can securely access your care plan, lab results, and other documents by logging into your member portal here: Log In
